Uniform Order Form

A combination of three tops and three bottoms only. Example: 2 short sleeve and 1 long sleeve. 2 pairs of shorts and 1 pair of pants. If the totals come to more than that, we will best guess what you want. This is the total you will get for employment so choose wisely.

**All uniforms must be returned upon termination of employment, or it will be deducted from your last paycheck.

By clicking SUBMIT you acknowledge that upon termination all items must be returned to your manager. If items are not returned, the cost will be deducted from your final paycheck.
